Description / Odor Incident Report

Instructions: This form is to be used for all odor complaints at or around the Valley Landfill site. Please complete all of the questions below. When you are done, please mail a copy of the report to Dallas Leonard, P.O. Box 452, Harrison City, PA 15636, or fax it to 724-744-7579.
